How Do You Submit a Letter of Authorization to Pick up a Passport?

Complete the following steps in order to submit your letter of authorization along with your expedited passport application:

Once you’ve finished filling out the form, make sure that you place it safely and securely inside the envelope that you plan on giving to the representative from your courier service. The representative will take the letter of authorization, along with all other documents inside the envelope and submit them to the Department of State on your behalf.
